I'm going to print out a bunch of these for work - I work for the City of Austin, and these are going to be REAL handy! Only problem is, most of the plaintiffs would be management!

We already have something like this in place at work. Only we call them an "Inmate Request" or "Form 303 - Formal Grievance" if the inmate didn't like the result of the "Informal Grievance" written on the "Inmate Request". Or if you're an officer it's an "Incident Report". And yes they get used much like how the "Hurt Feelings Report" is written.
